Every time I toggle the reader to Bluetooth mode and turn on my Bluetooth feature on my iPhone X, my NFC Actions app is unable to find the device. Is there something else I am missing here?

No it doesn’t seem you’re missing anything. You should be able to turn on the reader with NFC Actions open and it should automatically connect to it.
